I fully authorize Bucktown Visas & Passports to submit my United States passport application and to retrieve the passport on my behalf. I also authorize Bucktown Visas & Passports to discuss any and all matters pertaining to the issuance of my passport with the United States Department of State or the United States Passport Agency.

**If applicant is a minor, please print the child's name, and then one of the parents should sign and date this letter.
Please print out 2 letters of authorization and fill them out completely. Note: The clerk will put one letter inside of the sealed envelope, and you must keep the other letter on the outside of any government-sealed envelope. The same is true of the 2 copies of your travel itineraries. Your Bucktown Passport Order Form must be kept on the outside of any government-sealed envelope. (Sealed envelopes only apply to new passports, lost, stolen or damaged passports, and children's passports.)
